Optionalreason: stringDid not reach the audio. Prefer a Refusal — a code plus named args a client can localize;
the plain-string overload exists for producers not yet migrated and is log-only.
The two-argument form carries BOTH halves: the code the client renders, and the producer's own
sentence for the log. formatRefusal's CODE k=v rendering is precise and barely readable, so
a producer that already has a good diagnostic line ("the box on X has 8 preamp inputs — input
12 is beyond it") keeps it for the log without withholding the code from the wire. It is never
a way to put operator-facing prose on the wire: the client renders the CODE.
